Palos Health House Calls

The Palos House Call Program provides medical care in the comfort of your home. This service offers medical visits to individuals throughout our service area unable to leave their homes.

Our care team includes physicians, nurse practitioners, registered nurses and social workers. We provide compassionate medical care at home and in assisted and independent living facilities.

You may benefit from the program if:

  • You have had a recent hospitalization and are unable to follow up with your primary physician due to temporary limited mobility
  • You have a chronic illness such as heart disease, lung disease, kidney disease, diabetes or dementia

House Call visits may include:

  • A detailed medical history and exam
  • Individualized plan of care
  • Medical management
  • Assessment of additional needs at home
  • Prescriptions

Working with your physician

Our care team can work collaboratively with your primary care physician while House Call care is required. Our goal is to help physicians deliver care to patients that require medical visits and are unable to leave their home. Visits take place on an as needed basis.

As part of Palos Health, we provide cost-effective home health and rehabilitative services within the post-acute continuum. We offer a seamless transition back home for patients following acute or post-acute cases, helping you avoid unnecessary hospitalizations.

Most insurance plans cover House Calls

Medicare, Medicaid and most private insurance plans cover house calls. Please review your plan or call your insurance company to verify your coverage.
