Social Services at Palos Health

People trying to cope with the issues of serious illness, hospitalization and the need for medical care at home face many important decisions.

  • Where will I find the services I need?
  • Who will provide the support I need to get through this episode?
  • Who will look after those for whom I am responsible?
  • What role will my family and friends play in my care?

The Palos Health Social Services Department can help patients and families find answers to these and other important questions. Social Services staff provide medical social work services to help patients and their families adjust to the impact of illness and plan for meeting their continuing health care needs in the community.

Our services encompass three key areas: discharge planning, patient/family education and counseling.

Discharge Planning

Medical social workers work with a hospitalized patient’s family, as well as doctors, nurses and other health professionals, to arrange necessary services to help the patient return to the community setting, including making arrangements for:

  • Home care services
  • Rehabilitation therapy
  • Nursing home placement
  • Transportation
  • Medical equipment

Patient & Family Education

To be sure patients and families can confidently care for a sick or aging loved one and make appropriate medical decisions on their behalf, medical social workers provide education on:

  • Community resources
  • Advance directives (legal documents that specify how and by whom a person’s care will be managed if that person is unable to do so)


Medical social workers are specially trained professionals with the skills and compassion to provide assistance through extremely difficult times. They often counsel families about such issues as adjustment to a life threatening or debilitating illness, and end-of-life issues.
